# PMJAY covers 1350 procedures across 25 specialities including Cancer Care, Neurology, Nephrology etc.

# Benefit of upto Rs. 5 lakhs per eligible family per annum,

PMJAY is fast spreading its wings in our State
# 6 (Six) Medical Colleges (including State Cancer Institute) and 121 private hospitals currently empaneled under PMJAY are already providing medical services to our esteemed beneficiaries.

25 District Hospitals are now added to provide cashless procedures for all PMJAY beneficiaries from 5th November, 2018 onwards.

1.Q : What is the Amount covered under the Scheme?
Ans : Rs.5 lakhs covered per eligible family per year for Secondary and Tertiary care as cashless treatment on family floater basis.

2.Q : Who is eligible to avail the benefits of this Scheme?
Ans : All families listed in the SECC-2011 database as per defined criteria will be covered.

3.Q : Are there any reserved Packages for Government Hospitals?
Ans : Yes. Altogether 221 procedures are reserved for Government hospitals only.

4.Q : Do beneficiaries need to pay any fee for obtaining the card?
Ans : No, the card is free and no fee is to be paid for obtaining the card.

5.Q : How will the Beneficiary family get card for Cashless treatment?
Ans : Government is issuing the card to the beneficiary family directly as per SECC Data of the Year 2011 for availing Cashless Treatment.

6.Q : How many cards will be issued to a family?
Ans : Only one single card will be issued to one eligible family.

7.Q : How a beneficiary will get this PMJAY card, in case of lost or theft?
Ans : Beneficiary family will approach the Joint Director, Health with relevant proof of FIR to issue Duplicate Card, in their respective Districts.

8.Q : Where shall the beneficiary approach for support in empanelled hospitals?
Ans : Beneficiary should approach Arogya Mitra Help Desh at each of the empaneled hospital who will be there 24x7 for supporting the beneficiaries and ensuring that their treatement is in a seamless and cashless manner.

9.Q : Will any part of the treatment costs be reimbursable directly to the beneficiary?
Ans : No, patient will get cashless treatment in empaneled hospitals upto 5 lakhs per annum per family as per package benefit and the amount will be reimbursed to the hospitals directly. No cash will be paid to patients.

10.Q: What will happen it the total cost of treatment is above Rs.5 lakhs per family at any time or it Rs.5 lakhs get exhausted before one year for a beneficiary family?
Ans : In cash if the amount of Rs.5 lakhs is exhausted under PMJAY, then the follwing will occur:

   (a) Additional cost will be topped up form Atal Amrit Abhiyan Scheme (AAA) if the beneficiary is eligible under AAA and procedure is found covered under the Scheme with and additional amount of Rs.2 lakhs per person.

   (b) Otherwise additional amount has to be borne by the Beneficiary.

11.Q: Is there any restriction on family size for PMJAY?
Ans : No cap on family size and age of members.

12.Q: What additonal support is there for the beneficiaries in case of any grievance?
Ans : In case of any queries or grievance, a beneficiary can always call toll free number 18001027480 at anytime 24x7. Additionally,one can also send mail at grievance.nsc@gmail.com.

13.Q: Are pre-existing diseased covered under PMJAY?
Ans : Yes, all pre-existng diseases are covered under the Scheme and empanelled hospitals cannot deny treatment to the beneficiaries.

14.Q: What will be covered within Rs.5 lakhs annually?
Ans : The amount of Rs.5 lakhs for the beneficiary family per annum can be used by any enrolled member of the family on a floater basis as and when needed. This will cover the following:

Treatment: As per agreed package rates which will include pre and post hopitalization investigations and food for the patient

Travel Allowance and Daily Allowance: TA and DA will be paid to the patients as per State norms to the beneficiaries which are eligible under both Atal Amrit Abhiyan and Pradhan Mantri Jan Arogya Yojana.

Travelling Expenditure and Daily Allowance for treatment outside Assam will be paid only to those patients who are referred to outside Assam, hospitals for treatment through the referral system of SHA, Assam.

15.Q: What are the documents required at the time of Hospitalization?
Ans : At the time of Hospitalization within ans outside Assam, the beneficiary should show his/her PMJAY card along with one ID Proof (Voter ID/Pan Card/Licence etc.) or his/her name should come up in the database and his/her identity will be validated from his/her documents.

16.Q: "AAA" Card holder who are eligible but have not received PMJAY card. How they will get this card?
Ans : They can approach Ditrict Kiosk in every district to know about their status of PMJAY card or call the toll free number 18001027480.

17.Q: SECC Beneficiary who have not received "AAA" card, how will they get the "AAA" Card.
Ans : They can approach directly the District Kiosk of their respective Districts or get it from Mobile Enrollment Kiosk in their Blocks.

18.Q: Are Governemt employees eligible for the card?
Ans : Government employees (Central or State/Serving or Retired) are not eligible for benefit under the Scheme "PMJAY' or "Atal Amrit Abhiyan".
